Rehabilitation

Lutheran Care Center’s rehabilitation program offers comprehensive occupational therapy, physical therapy and speech language pathology services to residents admitted for short-term rehab, long-term care and those attending our Adult Day Health Care program.  Our rehab team collaborates daily with the other members of the interdisciplinary team to provide an individualized treatment plan needed to achieve optimal function in all aspects of self-care and mobility.

Our experienced team of rehab professionals treat people with a wide variety of needs including neurological illness or injury (stroke, TIAs, TBI, Parkinson’s Disease, MS, ALS), cardiovascular and cardio-pulmonary disease (MI, COPD, CHF, cardio-thoracic surgeries),  joint replacements and orthopedic injuries,  amputations, diabetes, pneumonia, recovery from surgeries, and other types of debility.  The department offers flexibility with residents and their families, providing therapy up to seven days a week, including holidays.

-  Ambulation & transfer training

-  Activities of daily living

-  Self-care management training

-  Pain management (including use of modalities when appropriate)

-  Cognitive assessment & retraining

-  Swallowing testing & therapy  (FEES -  in house skilled swallow study for dysphagia)

-  Speech & language therapy

-  Tracheotomy care

-  Discharge planning (including education with family & caregivers to ensure safe d/c plan)

-  Community integration

-  Home evaluations
